PSYHIATRIC CAUSE

  • Bipolar disorders: Suicide is common in untreated Bipolar disorders. 25 – 50% of patients attempt suicide at least once. Mixed state has increase suicidability.
  • Alcoholism
  • Psychotic Illness, such as Schizopherenia
  • Personality disorders
  • Severe to profound psychosocial stressors
  • Drug abuse

PHYSICAL ILLNESS

Increase prevalence in suicide ideation is seen in patients suffering from:

  • Viral fever
  • Hepatitis
  • Renal Failure
  • Mutilating injuries
  • Malignancy
  • Epilepsy

Drugs responsible for aggravating suicidal feeling are:

  • Reserpine (anti-hypertensive)
  • Phenothiazines
  • Barbiturates
  • Oral contraceptives

PSYCHOLOGICAL MECHANISM IN SUICIDE

Psychopathological suicide is displaced from of autoplastic aggression by a dependent or helpless person caught in a frustrating interpersonal situation. Teacher and Jacob described three stages whereby an adolescent comes to view suicide as the only solution to:

  • Long standing history of problems
  • A period of escalation of problem by introduction of new ones
  • A final stage, accent on onslaught of problems, normally characterized by a chain reaction which destroys any meaningful social relation.

There are three elements in suicide desire to kill, desire to be killed and desire to die. The escape from unbearable situation is a dominant theme in suicide and suicidal attempts. Duzkherm described three different social categories of suicide.

  • Egoistic due to loss of integration leading to sense of isolation and morbid individualism.
  • Altruistic, as seen in war and army – Men dying for country.
  • Anomic, where there is disturbance in balance of individuals aspiring on one hand and values of society on the other.

METHODS USED TO COMMIT SUICIDE

Methods used usually varies from time to time and place. Choice also depends on availability of methods. In developed countries, common methods used are:

  • Poisoning with domestic gas
  • Consumption of barbiturates
  • Use of fire arms and explosive
  • Hanging
  • Drowning

In recent times there is an increasing trend towards

  • Inflicting wounds and/or setting oneself on fire
  • Jumping in front of train or running vehicle
  • Excess consumption of drug such as sedatives / barbiturates or psychotropic drugs
